Background: Mylolysis is the loss of enamel and dentin at the cervical region of the tooth due to wear. It represents a challenging cervical lesion to treat.

Case Presentation: We report the clinical case of a 67-year-old man with type 2 diabetes mellitus who presented with bilateral bimaxillary mylolysis. Management required the extraction of tooth 3.6, bimaxillary scaling and root planing, and twenty-eight restorative treatments using hybrid composite resin.

Conclusion: Clinicians should keep this wear lesion in mind and take all necessary precautions during the different phases of dental treatment.

Pratical Implications: Clinicians should consider preoperative assessment, glycemic control, surgical precautions, and postoperative follow-up were essential elements to guarantee therapeutic success.
